The Brooklyn Center Community Schools Health Services Department strives to promote healthy life-styles and a healthy school environment. The Licensed School Nurses, along with the Health Services Assistants, provide quality health care, health maintenance and health promotion to the students. Our goal is to provide resolution of students’ health care needs that affect educational achievement.
Our first day of school for students is September 6th, 2016. 
 As we begin the 2016-2017 school year, we ask that every parent/guardian take the time to complete and return a HEALTH HISTORY FORM for each Brooklyn Center student.  We understand that health conditions and providers may change with each year. Providing updated information is the responsibility of the parents/guardians.  
For students entering Kindergarten, 7th grade, and 10th grade, please see the immunization page for the immunization schedule requirements. Immunization records must be provided to the School Health Office.  If you have any questions, please contact the school nurse:
